What is the Referral Process?
The Muslim Women Association (MWA), as the lead agency, has the responsibility for the overall case management of all referrals. As such, while a client may initially make contact with any of the Linking Hearts partners, all cases will be referred to the MWA Case Management team for initial assessment.
On assessing the client for suitability and needs the MWA Case Manager will then determine the appropriate interventions and refer aspects of the case management plan to any partners as required.
The MWA Case Manager will maintain the overall management of the client across the partnering team.
